Early Academic Alert Form

• Early Academic Alert is a program that allows faculty to provide academic feedback on student performance during the early weeks of classes, long before the mid-term point of the class.
• The intent is to give students a glimpse of their early performance and seek help (or adjust their habits) before it is too late to impact their final grade.

Student Success Action Plan (PDF)

• Use this template to create a plan and guidelines for a student who has displayed disruptive behaviors in your classroom or online environment and has not corrected those behaviors after previous discussions.
• The instructor and student complete the form together to ensure a mutually beneficial arrangement.
• The template provides brief instructions and examples in each area.
