Thanks for getting in touch :) Per your questions, I've seen this happen before when using the following method: 

• Rich Text: Using Edit > Click the Source Code button in the WYSIWYG Editor. 
  • This tends to play havoc with code. :/ 
    • How to use the rich text editor

Moving forward, the best two options I've found to be tried AND true are: 

• Avoid the WYSIWYG editor, and click the edit square > HTML
• Use a media widget instead.
